Job Description


Sugarman Health provides high quality person centred complex care for individuals within their own home. We are looking to expand our clinical accountability by building a team of clinicians to take a strategic lead on assessing and managing the packages of care we provide to our service users. We are currently seeking an adult nurse assessor to assess and plan the care for adults in the Community with complex health needs. They will provide leadership and management of the service we provide to the clients assigned to them. Ensuring adherence to nursing standards and all related policies, procedures, and guidelines


To conduct nursing assessments and produce detailed care assessments and care and support plans to ensure that our service users can remain safely in their own homes achieving the lifestyle and outcomes they want for themselves and their families.

This will include regular reviews of client care and management of staff to ensure they have necessary competences, liaising with other care professionals and providing feedback and reports to managers.

The role is 40 hours a week but this role requires the post holder to be flexible to ensure the care services are safely managed and will include participation in on-call rota.


* Ensure that professional practice standards and "best practice" are maintained in all areas of work
* To deliver productivity and quality standards agreed between yourself and the management team and to respond positively to feedback
* To undertake recorded assessments where required and plan with team to provide care
* To work unsupervised and use own initiative however know limitations and ask for support where necessary
* To analyse and interpret clinical information and medical evidence and provide a
* report in a professional and concise manner
* Work well as part of a multidisciplinary team and build positive working relationships
* Complete care plans, risk assessments
* Apply professional skills and manage own professional competence and accountability, in accordance with the NMC Code of Conduct
* In addition to the responsibilities listed above, the job holder may be required to perform other tasks as delegated
* Provide clinical support in an oncall diary


* Provide continued clinical input and support for staff in the daily delivery and support for individuals in their own home, ensuring that each person supported has their individual clinical care plans implemented and evaluated as required, in order to accommodate to their changing needs.

* Effectively delegate and supervise care staff in order to ensure all identified clinical and care needs are met.

* Have a clear understanding of personal accountability, undertake clinical procedures and training with individuals in accordance with care plans and sign off required competencies in a timely safe manner.

* Recognise and respond appropriately to urgent and emergency situations.

* Providing verbal and written reports/review on service issues and individual's progress to the Registered Manager.

* Complete training, competency assessments and supervision sessions for the teams allocated to the clinical services.

* Report real time incidents, accidents or complaints to the Manager in charge, in line with company policy and process and make necessary regulatory notifications in consultation with management.

* Record, audit and complete any appropriate documentation and records/systems in order to ensure that individual's records are accurate and informative.

* Communicate and coordinate with other Health and Social Care Professionals and Medical Practitioners, to ensure a comprehensive and cohesive clinical approach in all planned actions and interventions for the health needs of the individuals you support.


* Establish and maintain communication with people about routine and daily activities, overcoming any differences in communication between the people involve
* Demonstrate active listening skills
* Provide information and support to clients and their carers as applicable to the stage of the patients journey
* Manage emotional issues relating to the stages in the patients journey
* Maintain confidentiality, knowing when to disclose information appropriately to personnel and family members
* Facilitate and arrange meetings with service users and carers identified
* Accurately and comprehensively record information including sensitive and confidential information as appropriate
* Communicate with other health professionals and support agencies
* Recognise individual personal conflicts and take initial steps to resolve issues, whilst referring to a senior member of staff
* Attend nurse meeting and provide feedback on current case loads
* Relationship-building- understand and identify the training needs of carers and the best way teach .
* Mentoring-In this role, you'll guide-and teach and assess carers to provide care to service users in the Community and own homes


* To participate in effective teamwork.
* Identify and act in the clients best interest at all times, using effective decision making
* Responsible for assessment of allocated service users and setting up care
* Providing staff advice and support
* Provide spot checks to ensure standards are maintained and monthly reviews of all clients
* Provide information and support to clients and their carers as applicable to the stage of the patients journey

* Gains feedback on effectiveness of learning and how it can be improved
* Discusses own evaluation with the team and agrees on improvements for the future


* Plan, allocate and supervise the training of our carers and staff
* Communicates clearly with team members and gives them opportunity to contribute to the planning and organisation of work
* Organisation- assess and plan the implementation of care in the Community
* Relationship-building- discussing client needs with the Sugarman team and update
* Mentoring-In this role, you'll guide-and assess carers to provide care to service users in the community and own their own homes
* Assess their own and team work; respond to feedback
* Work with clinical educators to develop training plans and allocates work in a way which is consistent with team objectives, is realistic and achievable
* Takes full account of care staff abilities and development needs
* Objectively assess the work of the care staff and provides clear constructive feedback in a manner to maintain and improve team performance
* Provide carers and staff with feedback and actions plans to address training issues
* Regular feedback to Clinical Lead Nurse re staff competencies written and verbal
* Provide Sugarman staff with clinical advice and support


* Experience of working in the Community
* Excellent communication and organisational skills
* Strong person centered care
* Flexible to travel as required
* Thorough knowledge and understanding of clinical protocol, procedures, and standards within area of nursing expertise.
* Ability to assess service users and plan care


* Advanced nursing skills in complex care- Tracheostomy and ventilator competent
* Adult Nurse qualification
* Desire to maintain standards and participate in providing evidence based patient centred care
